An application and registration form should be submitted for each new student. Only a re-registration form should be submitted for current students who will be in grades K to 8 next year. You may request forms from the school office, or download them by clicking a link below:

You must also supply the following forms:

• Birth Certificate
• Baptismal Certificate (if applicable, unless baptized at St. John the Evangelist)
• Completed Immunization Form
